A partnership between Centura Health, Physician Health Partners (PHP) and Primary Physician Partners (PPP), CCHA combines extensive resources, key community resources, medical management and hundreds of primary care physicians to provide Coloradans with dedicated medical homes and quality healthcare options.
As healthcare reform moves forward, CCHA is committed to providing the right care, at the right time and in the right setting.
The Colorado Community Health Alliance is an Accountable Care Collaborative (ACC), a program designed to coordinate the healthcare of Medicaid members. Medicaid members in the ACC receive services using the fee-for-service model, and also belong to an “accountable care organization” that helps to manage their care. Starting in May 2011, some Medicaid clients will be automatically enrolled in the program or given the chance to opt out. Each region of the state has an Accountable Care Organization (ACC) called a Regional Care Collaborative Organization (RCCO). The RCCO helps to manage the care of the ACC members who live in the region it covers.
