CCHA Resources & Training for Providers

CCHA coordinates care and services for Health First Colorado members attributed to a primary care provider located in the following counties: Boulder, Broomfield, Clear Creek, El Paso, Gilpin, Jefferson, Park and Teller. We are also here to support contracted Health First Colorado providers with managing care for these members. If you are a participating provider, we offer a range of services and resources to help you and your practice.

The Department of Health Care Policy & Financing (HCPF) is phasing out administrative approvals beginning October 1, 2021 for private duty nursing (PDN) and pediatric long-term home health (PLTHH) benefits. CCHA will work with Case Management Agencies and HCPF to provide the following supports to members and providers during this implementation, and ongoing. Learn more here.

Care Coordination Escalation Path

CCHA providers must report all quality of care concerns and critical incidents that occur while a member is receiving services. Report these concerns using the CCHA Quality of Care Concern Notification Form. For more information, contact Providers@CCHAcares.com.
