Sometimes there are things in everyday life that affect your health and well-being. We can help you find resources to support you in living a healthy life. We work with many community agencies and assistance programs that provide a range of services.

Call us to talk with a CCHA care coordinator.

A care coordinator can provide information and refer you to resources for needs such as:

  • Food and housing
  • Transportation
  • Childcare
  • Job Placement
  • Financial or legal concerns
  • Mental health issues
  • Drug and alcohol abuse
  • Caregiver stress or burnout
