Audit of CCHA Performance for FY 2015 – 2016
Each year the Colorado Department of Health Care Policy and Financing engages Health Services Advisory Group, Inc. (HSAG) to conduct annual site reviews with each regional entity. The purpose is to assess successes and challenges in implementing key components of the Health First Colorado (Colorado's Medicaid Program) Accountable Care Collaborative (ACC).
CCHA Performance with ER Visits & Postpartum Checks
In the first three quarters of FY 2015 – 2016, CCHA has hit Key Performance Indicator (KPI) targets for ER Visits and Postpartum Checks. We believe our collaborative efforts to educate members has had an impact on continued reduction in ER Visits and an increased completeness rate for Postpartum Checks. The following table shows CCHA performance in the last month of each quarter to date.
